Please complete this form and return it with your check for $50, or credit card number and authorization for the charge in that amount. The $50 payment includes the $40 space charge for a 10’ x 15’ exhibit and selling space, plus a $10 refundable clean-up deposit. If you have removed all of your exhibit items and cleaned up your space at the end of the day, you will receive your $10 deposit back in cash. If not, the Lake Zurich Lions Club will retain the money to pay for park clean-up. All clean-up fees must be claimed by the end of the sale day. Provisions have been made to either donate unsold items to a charitable organization or to simply discard them. *All cleanup fees must be claimed by the end of the sale day.*

All proceeds from this event support the Lake Zurich Lions Club *Dollars for Scholars* Scholarship Fund. The Lions Club awards three scholarships each year, totaling more than $10,000, to individuals pursuing advanced education at colleges, universities or vocational schools. Although scholastic scores are important for these scholarships, the LZ Lions Club puts an even greater emphasis on the students’ sense of community involvement and financial need.

I agree that the Lake Zurich Lions Club shall not be held responsible or liable by me for any loss, theft or damage to any items exhibited, or for personal injury or loss of money during the "Biggest Yard Sale" held at Lions Park.
